The following considerations are summarised from Katherine Rachlin's small study in 1999 titled Factors Which Influence Individual’s Decisions When Considering Female-To-Male Genital Reconstructive Surgery of 23 individuals in New York City and San Francisco.

There are many reasons why men with a strong and stable male gender identity will not pursue the currently available genital surgery.

Factors influencing surgical decisions:

  • Contact and information from other men;
  • family obligations;
  • cost of multiple surgeries and time off work;
  • the extensive recovery time involved for numerous operations;
  • prohibitive health problems (such as diabetes);
  • risk of successful outcome;
  • technologically inadequate outcomes.

Many men choose to forego genital surgery because there is a high risk that surgery will not offer an affordable, realistic, and functioning phallus.

Some men still sacrifice everything to pursue the currently available surgically-constructed phallus.
